43. FACILITY USE REVOCABLE LICENSE AGREEMENT WITH BAYLESS HEALTHCARE GROUP AT THE SOUTH PHOENIX WIC SITE Approve full service Facilities Use/Revocable License Agreement (FURL) (P-50232) between Bayless Healthcare Group, Contractor, and Maricopa County, a political sub-division of the State of Arizona (County), by and through its Department of Public Health (MCDPH), Women, Infant and Children (WIC) Services to provide a developmental screening clinic (motor/social/emotional development and hearing/vision screening)to WIC eligible families. The Contractor has selected the MCDPH South Phoenix WIC facility at 438 East Southern, Phoenix, AZ 85040. The term of this Agreement is for two (2) months and shall commence August 7, 2013 to September 30, 2013. This FURL also allows for the Contractor use of clinic space at the South Phoenix WIC Site as scheduled on Exhibit “A”. The Contractor agrees to provide the services at no cost to the County and the recipients. County agrees to provide access without monetary remuneration. The Facility Review Committee will review and approve this FURL at the July 27 2013 meeting. (C-86-14-005-M-00)

59. RESCIND ACTION REGARDING FACILITIES USE REVOCABLE LICENSE AGREEMENT WITH BAYLESS HEALTHCARE GROUP FOR SPACE AT PUBLIC HEALTH WIC SITE, P-50232 Rescind the previously approved action item on August 7, 2013, (C-86-14-005-M-00) that approved a Facilities Use Revocable License Agreement (FURL) with Bayless Healthcare Group, for the purpose of providing a developmental screening clinic (motor/social/emotional development and hearing/vision screening) to WIC eligible families. Bayless faced "time" challenges, clientele not having any time to stay for appointments which typically can take an hour. This FURL was at no cost to either party. The Maricopa County Public Health WIC site is located at 438 E. Southern, Phoenix, AZ 85040. (C-86-14-005-M-01)
