64. PARTNERSHIP AGREEMENT WITH ST LUKES MEDICAL CENTER, LP FOR NURSE FAMILY PARTNERSHIP PROGRAM ENROLLMENT Approve Partnership Agreement (PA) between St. Luke’s Medical Center, LP, a Delaware Limited partnership doing business as (dba) Tempe St Luke’s Hospital, a Campus of St. Luke’s Medical Center dba The Women’s Care Center Clinic at Tempe St. Luke’s (WCC) and Maricopa County through its Department of Public Health (MCDPH), Nurse Family Partnership program (NFP) to provide three office spaces, free of charge, for NFP to conduct client enrollment. The PA term will begin September 1, 2013 and end on August 31, 2014, with an auto-renewal for successive one year terms, without the necessity of notice action by either party, however may be terminated in writing by either party.
