70. INTERGOVERNMENTAL AGREEMENT WITH ARIZONA DEPARTMENT OF HEALTH SERVICES FOR EMPOWER PLUS PROGRAM Approve Intergovernmental Agreement (IGA) Contract No. (ADHS14-052676) between Arizona Department of Health Services (ADHS) and Maricopa County by and through its Department of Public Health (MCDPH), Community Health Nursing (CHN) to provide funding for the Empower PLUS program. The Empower Plus program will assist Early Care and Education providers in improving the quality of care they provide to young children with respect to nutrition, breastfeeding support, physical activity and screen time using a learning collaborative method. The price sheet is for a not-to-exceed amount of $83,880, effective September 1, 2013 through August 31, 2014. Also, the term is September 1, 2013 through August 31, 2018. The Department of Public Health indirect rate for FY2013-14 is 15%. The indirect costs are estimated at $10,941, which are fully recoverable. Grant revenues are not local revenues for the purpose of the constitutional expenditure limitation, and therefore expenditure of the revenues is not prohibited by the budget law. This Amendment does not alter the budget constraining expenditures of local revenues duly adopted by the Board pursuant to A.R.S. §42-17105. The overall grant budget will be adjusted as necessary to accommodate this grant through a future reconciliation. Funding for this Agreement is provided by a Grant from ADHS and will not affect the County’s general fund. (C-86-14-026-3-00) 71. AGREEMENT WITH THE CITY OF CHANDLER FOR STRATEGIC NATIONAL STOCKPILE ASSETS Approve the Agreement between the City of Chandler and Maricopa County through its Department of Public Health (MCDPH); Office of Preparedness Response (OPR), to provide Strategic National Stockpile (SNS) Assets in the event of a public health emergency. The term of this non-financial agreement shall begin upon execution by both parties and shall be in effect until terminated by either party. The MCDPH, Office of Preparedness Response was previously known as the Maricopa County Department of Public Health’s Emergency Management Program (PHEM). (C-86-14-025-3-00)

51. AMENDMENT TO IGA WITH ARIZONA DEPARTMENT OF HEALTH SERVICES FOR EMPOWER PLUS PROGRAM Approve an Amendment to Intergovernmental Agreement (IGA) Contract No. (ADHS14-052676) between Arizona Department of Health Services (ADHS) and Maricopa County by and through its Department of Public Health (MCDPH), Community Health Nursing (CHN) to provide funding for the Empower PLUS program. Pursuant to Terms and Conditions, Provision 6, Contract changes, Item 6.1, Amendments, Purchase Orders and Change Orders, the Price Sheet of the Agreement is revised and replaced with the Price Sheet of this Amendment One, on page seven (7) and Item 6.1 Scope of Work, Section 3, Tasks Section 4.1 is revised and replaced per page one (1) of this Amendment One. The price sheet is amended to add $2,400 for Trainer Preparation Meetings for a contract total of $86,280, effective August 1, 2014. The term of this Agreement is September 1, 2013 through August 31, 2018. The Department of Public Health indirect rate for FY2015 is 14.8%. The Department’s indirect rate for FY2013-14 was 15%. The indirect costs are estimated at $313.00, which are fully recoverable. Grant revenues are not local revenues for the purpose of the constitutional expenditure limitation, and therefore expenditure of the revenues is not prohibited by the budget law. This Amendment does not alter the budget constraining expenditures of local revenues duly adopted by the Board pursuant to A.R.S. §42-17105. The overall grant budget will be adjusted as necessary to accommodate this grant through a future reconciliation. AMENDMENT TO IGA WITH ARIZONA DEPARTMENT OF HEALTH SERVICES FOR EMPOWER PLUS PROGRAM Approve a retro-active Purchase Order to Intergovernmental Agreement (IGA) Contract No. (ADHS14-052676:2) between Arizona Department of Health Services (ADHS) and Maricopa County by and through its Department of Public Health (MCDPH), Community Health Nursing (CHN) to provide funding for the EmpowerPLUS Program. The funding is in the not-to-exceed amount of $101,200.00 for the budget period of October 1, 2014 through September 30, 2015. The term of this Agreement is September 1, 2013 through August 31, 2018. All other terms and conditions remain in full force and effect. The Department of Public Health’s indirect rate for FY15 is 14.8%. The indirect costs are estimated at $13,047 which are fully recoverable. Grant revenues are not local revenues for the purpose of the constitutional expenditure limitation, and therefore expenditure of the revenues is not prohibited by the budget law. This Amendment does not alter the budget constraining expenditures of local revenues duly adopted by the Board pursuant to A.R.S. §42-17105. The overall grant budget will be adjusted as necessary to accommodate this grant through a future reconciliation.
