42. ELECTRONIC TRANSMISSION AGREEMENT WITH BANNER HEALTH, AN AZ NONPROFIT CORP FOR CONFIDENTIAL PATIENT IDENTIFIABLE INFORMATION Approve non-financial Electronic Transmission Agreement between Banner Health, an Arizona nonprofit Corporation (BH) and Maricopa County by and through its Department of Public (MCDPH), Disease Control to provide access to certain confidential patient identifiable information available electronically. BH will allow MCDPH access, and to permit its physicians, healthcare provides, and employees to access, Patient Information to provide patient care services. This agreement shall be effective upon signature by all parties and shall remain in effect until terminated by either party for any reason by written notice to either party; provided however, that the provision of Paragraphs 2, 5,7 and 10 shall survive the termination of this agreement.
