51. FACILITY USE REVOCABLE LICENSE AGREEMENTS P50238 WITH KEOGH HEALTH FOR OUTREACH SERVICES Approve full service Facilities Use/Revocable License Agreement (FURL) (P50238) between Keogh Health Connection (KHC), Contactor, and Maricopa County, a political sub-division of the State of Arizona (County), by and through its Department of Public Health (MCDPH), Women, Infant and Children (WIC) Services to provide assistance with enrollment into Department of Economic Services (DES), Arizona Health Care Cost Containment Systems (AHCCCS), Food Stamps and Cash Assistance services to underserved WIC eligible children ages zero (0) to five (5), their families and pregnant women. The term of this Agreement is for two (2) years and shall commence January 29, 2014 through January 28, 2015. Also, KHC will provide assistance at eight (8) County owned spaces as indicated on Exhibit “A” as follows: 1). Glendale WIC Clinic, 5141 W. Lamar Rd, Glendale, AZ 85301 2). St. Mary’s WIC Clinic, 4002 N. 67th Ave, Phoenix, AZ 85017 3). Downtown WIC Clinic, 1645 E. Roosevelt, Phoenix, AZ 4). Broadway WIC Clinic, 635 E Broadway Rd, Mesa, AZ 85204 5). Greenfield WIC Clinic, 4419 E. Main St, Mesa, AZ 85205 6). Mesa WIC Clinic, 430 N. Dobson Rd., #111, Mesa, AZ 85021 7). Chandler WIC Clinic, 3002 N Arizona Ave. #13, Chandler, AZ 85225 8). Guadalupe WIC Clinic, 9206 S Avenida del Yaqui, Guadalupe, AZ 85283 This FURL also allows for the Contractor use of clinic spaces scheduled on Exhibit “A”. The Contractor agrees to provide the services at no cost to the County and the recipients. County agrees to provide access without monetary remuneration. (C-86-14-057-M-00)
