52. FACILITY USE REVOCABLE LICENSE AGREEMENT FOR P50246 WITH EMPOWERMENT SYSTEMS, INC. FOR OUTREACH SERVICES Approve full service Facilities Use/Revocable License Agreement (FURL) (P50246) between Empowerment Systems, Inc. (ES), Contactor, and Maricopa County, a political sub-division of the State of Arizona (County), by and through its Department of Public Health (MCDPH), Women, Infant and Children (WIC) Services to provide assistance with enrollment into Kids Health Links Program to underserved WIC eligible children ages zero (0) to five (5), their families and pregnant women. The term of this Agreement is for two (2) years and shall commence January 29, 2014 through January 28, 2016. The Contractor has selected six (6) County owned spaces as follows: 1).Greenfield WIC Clinic, 4419 E. Main St, Mesa, AZ 85205 2). Mesa WIC Clinic, 430 N. Dobson Rd., #111, Mesa, AZ 85021 3). Sunnyslope WIC Clinic, 8838 N. Central Ave, Ste 100, Phoenix, AZ 85020 4). Downtown WIC Clinic, 1645 E. Roosevelt, Phoenix, AZ 5). North Valley WIC Clinic, 19401 N Cave Crk Rd, Ste 8, Phoenix, AZ 85024 6). St. Mary's WIC Clinic, 3003 W. Thomas Rd, Phoenix, AZ 85017 This FURL also allows for the Contractor use of clinic spaces scheduled on Exhibit “A”. The Contractor agrees to provide the services at no cost to the County and the recipients. County agrees to provide access without monetary remuneration. (C-86-14-060-M-00)
