56. PROVIDER PARTICIPATION AGREEMENT WITH HEALTH NET OF ARIZONA Approve a Provider Participation Agreement between Health Net of Arizona (HNAZ) and Maricopa County by and between its Department of Public Health (MCDPH), dba Maricopa County Department of Health Services dba Public Health Clinical (Laboratory), Maricopa County dba Maricopa County Department of Health Services dba Public Health Services dba Healthcare for the Homeless Clinic for the registration and payment for health care services provided by Public Health Clinic for fee-for service eligible persons. The term of this Agreement shall begin upon Board of Supervisors approval and shall remain in effect until terminated by either party with a 30-day notice. This Agreement allows Maricopa County Department of Public Health, dba Maricopa County Department of Health Services dba Public Health Clinical and Healthcare for the Homeless to bill Health Net of Arizona (HNAZ) health plan for reimbursable client services provided at the Public Health Clinic Laboratory and the Homeless Clinic. This allows Public Health Clinic Laboratory and Healthcare for the Homeless, a federally grant funded programs, to generate additional revenue for the program. The overall fee fund budget will be adjusted as necessary to accommodate increase in revenue through a future reconciliation. (C-86-14-072-3-00)
