49. LICENSE AGREEMENT WITH AMERICAN DENTAL ASSOCIATION Approve License Agreement between American Dental Association (ADA) (Licensor) and Maricopa County by and through its Department of Public Health (MCDPH), Office of Oral Health (OOH) (Licensee), that Licensee is a provider of continuing dental education activities that desire to use the ADA Continuing Education Recognition Program (CERP) trademark/service mark and/or name to advise potential course participants of the recognition of their continuing dental education activities by Licensor. The license recognition term is from November 1, 2013 through December 31, 2015 for a fee amount of $350 per year for a total not exceed amount of $700.00. The term of this Agreement shall be concurrent with term of recognition granted by the Licensor. (C-86-14-073-3-00)

47. ADMINISTRATIVE CORRECTION TO LICENSE AGREEMENT WITH AMERICAN DENTAL ASSOCIATION Approve an Administrative Correction to the agenda item regarding License Agreement between American Dental Association (ADA) (Licensor) and Maricopa County by and through its Department of Public Health (MCDPH), Office of Oral Health (OOH) (Licensee), originally approved on January 29, 2014. (License Agreement, agenda item C-86-14-073-3-00). This Administrative Correction provides information regarding the Licensee Agreement in addition to that previously provided for certification of dental education and use of ADA Continuing Education Recognition Program (CERP) Mark and the Licensor fee of $350 per year for a total of $700 for the license recognition term from November 1, 2013 through December 31, 2015. The Administrative Correction is the Licensor fee is being replaced with the following fee schedule; $390 per year for a total of $780, plus an initial Application for Recognition fee of $735.
