34. AMENDMENT TO FACILITY USE/REVOCABLE LICENSE AGREEMENT WITH KEOGH HEALTH CONNECTION Approve, and authorize the Chairman to execute Amendment No. 4 to License Agreement P-50238 with Keogh Health Connection, an Arizona non-profit corporation, (“Contractor”) to extend the term of the Agreement for approximately two years. The Agreement is for Contractor use of County Public Health Women and Infant Children (WIC) space to provide assistance with enrollment into Department of Economic Services (DES), Arizona Health Care Cost Containment Systems (AHCCCS) Supplemental Nutrition Assistance Program (SNAP) and Cash Assistance services to underserved WIC eligible children ages zero (0) to five (5) years, their families and pregnant women. This amendment will extend the term two years commencing March 1, 2020 and expire February 28, 2022. There is no cost to the County. Either Party may terminate the Agreement via 30-day written notice to the other. The number of sites is reduced from four to one.
