35. AMENDMENT TO FACILITY USE/REVOCABLE LICENSE AGREEMENT WITH EMPOWERMENT SYSTEMS, INC. Approve, and authorize the Chairman to execute Amendment No. 3 to License Agreement P-50246 with Empowerment Systems, Inc., an Arizona non-profit corporation, (“Contractor”) for continued use of County controlled property at 1645 E. Roosevelt in Phoenix. The Agreement is for Contractor use of County Public Health, Women and Infant Children (WIC) space to provide assistance with enrollment into Kids Health Links Program to underserved WIC eligible children ages zero (0) to five (5) years, their families and pregnant women. This amendment will extend the term two years commencing March 1, 2020 and expire February 28, 2022. There is no cost to the County. The one location is at 1645 E. Roosevelt Street, Phoenix 85006.
