AFFILIATION AGREEMENT WITH CREIGHTON ELEMENTARY DISTRICT #14 FOR CLINICAL NUTRITION TRAINING EXPERIENCE Approve the Affiliation Agreement between Creighton Elementary District #14 (CED) and Maricopa County through its Department of Public Health (MCDPH) to provide clinical nutrition training experience for graduate students in the dietetic internship program. The agreement is non-financial, and the term is April 1, 2014 through June 30, 2018. (C-86-14-100-3-00)
