31. AFFILIATION AGREEMENT ENTITLED, “ADJUNCT AFFILIATION AGREEMENT FOR MEDICAL EDUCATION” WITH DIGNITY HEALTH D/B/A ST. JOSEPH’S HOSPITAL AND MEDICAL CENTER Approve the Affiliation Agreement entitled, “Adjunct Affiliation Agreement for Medical Education” between Dignity Health d/b/a St. Joseph’s Hospital and Medical Center (DHSJHMC) and Maricopa County Department of Public Health (MCDPH) to allow residents to participate in learning experiences at the MCDPH. The agreement is non-financial and the term is from the last date on which the Agreement has been executed by both Parties (the “Effective Date”) and expires five (5) years after the Effective Date, subject to the termination provisions of the Agreement. 48. AMENDMENT TO ADJUNCT AFFILIATION AGREEMENT WITH DIGNITY HEALTH D/B/A ST. JOSEPH’S HOSPITAL AND MEDICAL CENTER TO RECOGNIZE A NAME CHANGE Approve this non-financial amendment to the Adjunct Affiliation Agreement for Medical Education between Dignity Health, a California nonprofit public benefit corporation d/b/a St. Joseph’s Hospital and Medical Center (SJHMC) and Maricopa County by and through its Department of Public (MCDPH) to recognize a name change to the “Sponsoring Organization” to Dignity Community Care (DCC), a California nonprofit Corporation d/b/a SJHMC. This agreement allows residents to participate in learning experiences at the MCDPH. All other terms and conditions of the original affiliation agreement shall remain in full force and effect.
