84. IGA WITH AVONDALE ELEMENTARY SCHOOL DISTRICT FOR SCHOOL-BASED TOBACCO USE PREVENTION AND EDUCATION SERVICES Approve an Intergovernmental Agreement (IGA) between Avondale Elementary School District (AESD) and Maricopa County by and through its Department of Public Health (MCDPH) to provide School-based Tobacco Use Prevention and Education Services; School Health Index (SHI). Funding is in the not-to-exceed amount of $1,000 for the budget period beginning August 1, 2013 through May 1, 2014. Avondale Elementary School District thought they had submitted their contract but realized they hadn't. We are reimbursing them at this point for the money spent per the contract. The dates of service are correct 8/1/13 through 5/1/14. (C-86-14-106-3-00)
