83. IGA WITH ARIZONA DEPARTMENT OF HEALTH SERVICES FOR HYPERTENSION AND DIABETES SYSTEM OF CARE Approve an Intergovernmental Agreement (IGA) ADHS14-072293 between Arizona Department of Health Services (ADHS) and Maricopa County by and through its Department of Public Health (MCDPH) to provide funding for the Hypertension and Diabetes System of Care Program. The budget is in the not-to-exceed amount of $182,184 for the budget period of May 1, 2014 through June 30, 2015. The Department of Public Health’s indirect rate for FY14 is 15%. Grant indirect costs are 15% of salaries and employee related expenses for a total of $149,489. The full indirect costs are estimated at $23,763 of which $22,423 is allowable and $1,340 is not allowable. Grant revenues are not local revenues for the purpose of the constitutional expenditure limitation, and therefore expenditure of the revenues is not prohibited by the budget law. This Amendment does not alter the budget constraining expenditures of local revenues duly adopted by the Board pursuant to A.R.S. §42-17105. The overall grant budget will be adjusted as necessary to accommodate this grant through a future reconciliation. Funding for this Agreement is provided by a Grant from ADHS and will not affect the County’s general fund. (C-86-14-108-3-00)

56. AMENDMENT TO IGA WITH ARIZONA DEPARTMENT OF HEALTH SERVICES FOR HYPERTENSION AND DIABETES SYSTEM OF CARE Approve an Amendment to an Intergovernmental Agreement (IGA) ADHS14-072293 between Arizona Department of Health Services (ADHS) and Maricopa County by and through its Department of Public Health (MCDPH) to provide funding for the Hypertension and Diabetes System of Care Program. This Amendment replaces Page Seventeen (17), Price Sheet Effective May, 2014 in the not-to-exceed amount of $ 182,184. Per the contract, 1. Itemized Service Budget (ISB) Restrictions: 1.1 The Contractor is authorized to transfer up to a maximum of ten percent (10%) of the total Contract amount among the categorical line items. Transfers of funds are only allowed among funded line items as detailed in the original ISB. Transfers exceeding ten percent (10%) of the Contracted amount, or to a non-funded line item, shall require a Contract Amendment. The Department of Public Health’s indirect rate for FY15 is 14.8%. Grant indirect costs are 15% of salaries and employee related expenses for a total of $6,301. The full indirect costs are estimated at $23,487 of which $945 is allowable and $22,542 is not allowable. Maricopa County Department of Public Health (MCDPH) sub-contracted to Native Health who was a successful respondent to a Notice of Intent issued by MCDPH pursuant to PH NOI 15-002. This Contract was procured under Section MC1-330 of the Maricopa County Procurement Code, Request for Proposal. The changes to the Price Sheet reflect the necessary changes needed in order to sub-contract with Native Health. (C-86-14-108-3-01)

60. AMENDMENT TO IGA WITH ARIZONA DEPARTMENT OF HEALTH SERVICES FOR HYPERTENSION AND DIABETES SYSTEM OF CARE Approve a retro-active Amendment No. 2 to an Intergovernmental Agreement (IGA) ADHS14-072293 between Arizona Department of Health Services (ADHS) and Maricopa County by and through its Department of Public Health (MCDPH) to provide funding for the Hypertension and Diabetes System of Care Program. This Amendment adds to the Scope of Services, Provision 4. Tasks, 4.14 Conduct needs assessment and site reviews of other area clinics for future program expansion. Scope of Services, Provision 5. Deliverables and Delivery Schedule is amended to add 5.3.6 Written needs assessment review of other local clinics and recommendations for future program expansion due by June 29, 2015. This Amendment replaces the Price Sheet effective March 1, 2015 through June 29, 2015 which was increased by $15,938 in the not-to-exceed amount of $100,938.00. Terms and Conditions, Provision 17. Health Insurance Portability and Accountability Act of 1996, delete Sections 17.1 and 17.2 and replace with page 2 of this Amendment. The Department of Public Health’s indirect rate for FY15 is 14.8%. Grant indirect costs are 15% of salaries and employee related expenses for a total of $12,989. The full indirect costs are estimated at $2,055 of which $1,948 is allowable and $107 is not allowable. Maricopa County Department of Public Health (MCDPH) sub-contracted to Native Health who was a successful respondent to a Notice of Intent issued by MCDPH pursuant to PH NOI 15-002. This Contract was procured under Section MC1-330 of the Maricopa County Procurement Code, Request for Proposal. The changes to the Price Sheet reflect the necessary changes needed in order to sub-contract with Native Health. (C-86-14-108-3-02)
