85. CONTRACT WITH CITY OF PHOENIX FOR TB SCREENING AND DIAGNOSTIC SERVICES FOR HEAD START PROGRAM AND EARLY HEAD START PROGRAM Approve a Contract with the City of Phoenix, Human Services Department and Maricopa County by and through its Department of Public Health (MCDPH), Clinical Services. This Contract is to establish an account so the City of Phoenix can make referrals of parents or children who need diagnostic services for tuberculosis, including TB skin tests. The budget is in the not-to-exceed amount of $4,000 for the budget period of July 1, 2013 through June 30, 2014 with the option to renew for two (2) additional one-year periods. The Contract inadvertently was not previously approved by the Board of Supervisors. (C-86-14-109-3-00)

24. RESCIND AND APPROVE CONTRACT AMENDMENT WITH CITY OF PHOENIX FOR TB SCREENING AND DIAGNOSTIC SERVICES Rescind Board Action of June 25, 2014, agenda item C-86-14-109-3-01 approving Amendment No. 1 with City of Phoenix (COP) which added additional term language and the Amendment was not signed. Approve revised Amendment No. 1 between the City of Phoenix, Human Services Department and Maricopa County by and through its Department of Public Health (MCDPH), Clinical Services. This Amendment is to establish an account so the City of Phoenix can make referrals of parents or children who need diagnostic services for tuberculosis, including TB skin tests. The budget is in the not-to-exceed amount of $4,000 for the budget period of July 1, 2014 through June 30, 2015 for a total not-to-exceed amount of $8,000. Also, remove in its entirety item 1.7, Iran and Sudan, under Terms and Conditions; Revise under Insurance Requirements, item C. Notice of Cancellation and item E., Verification of Coverage. 80. AMENDMENT TO CONTRACT WITH CITY OF PHOENIX FOR TB SCREENING AND DIAGNOSTICS Approve Amendment No. 2 Contract (No. 136014) TB Screening and Diagnostic between the City of Phoenix, Human Services Department and Maricopa County by and through its Department of Public Health (MCDPH), Clinical Services, Tuberculosis (TB) Program to provide funding for TB Services. This Amendment is to establish an account so the City of Phoenix can make referrals of parents or children who need diagnostic services for tuberculosis, including TB skin tests. This contract amendment includes the following revisions: 1) In accordance with the contract term, this contract is being renewed for the period July 1, 2015 to June 30, 2016, which is the 2nd and final renewal. 2) For the renewal period, the funding being added is $4,000. The total cumulative amount for the period of July 1, 2013 to June 30, 2016 is $12,000. 3) City contact information under “Notices” is being revised. All other terms and conditions of the original contract not specifically modified herein or in conflict with the Amendment shall remain unchanged and in full force and effect. This amendment shall become effective on the date of the last signature unless otherwise specified herein. (C-86-14-109-3-03)

AMENDMENT TO CONTRACT WITH CITY OF PHOENIX FOR TB SCREENING AND DIAGNOSTIC SERVICES FOR HEAD START PROGRAM AND EARLY HEAD START PROGRAM Approve an Amendment No. 3 to renew the Contract with the City of Phoenix, Human Services Department and Maricopa County by and through its Department of Public Health (MCDPH), Clinical Services. This will continue the partnership whereby the City of Phoenix makes referrals of parents or children who need diagnostic services for tuberculosis, including TB skin tests. The budget is in the not-to-exceed amount of $5,000 for the budget period of July 1, 2016 through June 30, 2017. The term is for 3 years (July 1, 2016 through June 30, 2019) for a not-to-exceed total amount of $15,000. (C-86-14-109-3-04)
