85. REVOCABLE LICENSE AGREEMENT P-50251 WITH CENTRAL ARIZONA SHELTER SERVICES, INC Approve, and authorize the Chairman to execute, Revocable License Agreement P-50251 with Central Arizona Shelter Services, Inc., an Arizona non-profit (the “Licensee”) for Licensee’s restricted use of space (the “Premises”) within the County’s Health Care for the Homeless facility at the Human Services Campus at 230 S. 12th Avenue in Phoenix, Arizona. The term of this agreement shall be for one (1) year and shall commence on August 1, 2014 and shall expire on July 31, 2015. License Agreement P-50251 permits the Licensee use of the Premises for one year with an option delegated to the Real Estate Manager to renew the License for no more than one additional year provided there is no additional cost to the County. The Premises will be used to facilitate staff from the Veterans Administration to provide counseling services, employment services, and/or case management services to veterans experiencing homelessness. The Licensee will be responsible for complying with all federal, state and local laws, statutes, rules, regulations, ordinances codes and executive orders including Health Insurance Portability and Accountability Act of 1996 (HIPPA) compliance.
