32. AGREEMENT WITH CHICANOS POR LA CAUSA, INC., PARENTING ARIZONA FOR WIC ENROLLMENT SERVICES Approve a non-monetary Agreement between Chicanos Por La Causa, Inc., Parenting Arizona (CPLC PA) and Maricopa County by and through its Department of Public Health (MCDPH), Women, Infant and Children Services (WIC) to provide WIC enrollment services to the WIC eligible clients at CPLC PA located at 5834 E Calle Santos Bravo, Guadalupe, AZ 85283. The Agreement is part of the WIC Services grant. The term of this Agreement shall be for five (5) years and shall commence on October 1, 2014 to September 30, 2015. Thereafter, this Agreement will be automatically renewed for successive one year terms, without the necessity of notice action by either party, however may be terminated as provided by options with the Agreement. Either party may terminate this Agreement upon 30 days prior written notice. (C-86-15-012-3-00)
