30. LEASE AGREEMENT WITH ST. MARY’S FOOD BANK ALLIANCE, INC. Approve, and authorize the Chairman to execute, Lease Agreement No. L-7455 with St. Mary’s Food Bank Alliance, Inc., an Arizona non-profit corporation, for the County’s use of approximately 6,400 square feet of office and clinic space at 3003 West Thomas Road, Phoenix. Public Health will use the subject leased space for the continued operation of a regional WIC facility. The term of Lease L-7455 will be for 36 months beginning November 1, 2014 and expiring on October 31, 2017, unless terminated earlier as provided for in the agreement. The monthly base rent during the lease term will be $6,000.00 per month ($11.25 per square foot annually) plus rental tax. 37. AMENDMENT TO LEASE AGREEMENT L-7455 WITH ST. MARY’S FOOD BANK ALLIANCE Approve, and authorize the Chairman to execute Amendment No. 2 to Lease Agreement No. L-7455 with St. Mary’s Food Bank Alliance, an Arizona non-profit corporation, for County use of approximately 6,400 square feet of office and clinic space at 3003 West Thomas Road, Phoenix and, delegate authority to both the Deputy County Manager for Maricopa County and the Maricopa County Real Estate Director to perform administrative actions, including execution of documents, to periodically carry out the intent of the Lease. Public Health will use the subject leased space for the continued operation of a regional WIC facility. The amendment extends the lease term for 12 months commencing November 1, 2018 and expiring on October 31, 2019, unless terminated earlier as provided for in the agreement. The monthly lease rate will reduce by $200.00 per month from the previous year. The monthly base rent during the lease term will be $5,800.00 per month ($10.85 per square foot annually) plus rental tax. The County will be responsible for providing janitorial services to the leased premises.
