76. AMENDMENT TO LEASE AGREEMENT L-7455 WITH ST. MARY’S FOOD BANK ALLIANCE Approve Amendment No. 4 to Lease Agreement No. L-7455 with St. Mary’s Food Bank Alliance, an Arizona non-profit corporation, for County use of approximately 6,400 square feet of office and clinic space at 3003 West Thomas Road, Phoenix. Maricopa County Public Health will use the subject leased space for the continued operation of a regional WIC facility. The amendment extends the lease term for one (1) year commencing November 1, 2020 and expiring on October 31, 2021, unless terminated earlier as provided for in the agreement. The monthly base rent during the lease term will remain at $5,800.00 per month ($10.88 per square foot annually) plus rental tax. Estimated annual cost is $71,618.40. The County will be responsible for providing janitorial services to the leased premises. The County may terminate the Lease by providing a 90-day written notice, non-appropriation of funds notice and pursuant to A.R.S. § 38-511. (C-86-15-016-1-04)

55. AMENDMENT TO LEASE AGREEMENT L-7455 WITH ST. MARY’S FOOD BANK ALLIANCE Approve and execute Amendment No. 5 to Lease Agreement L-7455 with St. Mary’s Food Bank Alliance, for County (“Lessee”) use of 6,400 square feet of office and clinic space at 3003 West Thomas Road, Phoenix. The purpose of this amendment is to extend the term of the Agreement, at the current rate, for one (1) year commencing on November 1, 2021 through October 31, 2022, unless terminated earlier as provided for in the agreement. The monthly base rent during the lease term will remain at $5,800.00 per month plus rental tax. The Agreement is subject to termination pursuant to the provisions of A.R.S. 38-511. The Lessee may terminate the Lease by providing a 90-day written notice to Lessor or at the end of any fiscal year due to non-appropriation of funds without any penalty or liability to Lessee. MARY'S FOOD BANK ALLIANCE FOR OFFICE AND CLINIC SPACE LOCATED AT 3003 WEST THOMAS ROAD, PHOENIX Approve and execute Amendment 6 to Lease Agreement, Lease L7455, between St. Mary’s Food Bank Alliance and Maricopa County for the use of 6,400 square feet of office and clinic space located at 3003 West Thomas Road, Phoenix. 80. AMEND LEASE AGREEMENT WITH ST. MARY'S FOOD BANK ALLIANCE Approve and execute the Seventh Amendment to Lease Agreement between St. Mary’s Food Bank Alliance and Maricopa County for the use of 6,400 square feet of office and clinic space located at 3003 West Thomas Road in Phoenix. This amendment extends the term of the Lease two years, from November 1, 2024 through October 31, 2026, but allows early termination by Landlord by giving 180-days prior written notice no sooner than May 1, 2025. The monthly base rent during the extended lease term escalates 3% annually. This leased property is in Supervisor District 5. (C-86-15-016-1-07)
