50. FACILITIES USE AGREEMENT WITH ARIZONA BOARD OF REGENTS Approve, and authorize the Chairman to execute Facilities Use Agreement P-50244 between Arizona board of Regents, for and on behalf of (1) Arizona State University (ASU), (2) Northern Arizona University (NAU), and (3) University of Arizona (UA), (the “Contractor”) and Maricopa County for Contractor’s exclusive use of space (the “Premises”) within the County’s Health Care for the Homeless facility at the Human Services Campus at 220 S. 12th Avenue in Phoenix, Arizona. Facilities Use Agreement P-50244 permits the Licensee exclusive use of the Premises on Saturdays and Sundays between 8:00AM and 5:00PM and weekday evenings as may be approved by the County in its reasonable discretion, for one year with an option, delegated to the Real Estate Manager, to renew the License for no more than one additional year provided there is no additional cost to the County. The Premises will be used to facilitate staff from the Contractor to provide a primary care clinic providing primary healthcare and primary mental health services to those experiencing homelessness. The Contractor will be responsible for complying with all federal, state and local laws, statutes, rules, regulations, ordinances codes and executive orders including Health Insurance Portability and Accountability Act of 1996 (HIPPA) compliance. The County will be responsible to provide Clinic Supplies and Medications. The Contractor will not be charged a license fee. (C-86-15-028-M-00)

66. AMENDMENT TO FACILITIES USE AGREEMENT WITH ARIZONA BOARD OF REGENTS FOR USE OF SPACE AT HUMAN SERVICES CAMPUS Approve and authorize the Chairman to execute amendment 2 to Facilities Use Agreement P-50244 between Arizona Board of Regents, for and on behalf of (1) Arizona State University (ASU), (2) Northern Arizona University (NAU), and (3) University of Arizona (UA), (the “Contractor”) and Maricopa County for Contractor’s exclusive use of space (the “Premises”) within the County’s Health Care for the Homeless facility at the Human Services Campus at 220 S. 12th Avenue in Phoenix, Arizona. In addition, the term of this Agreement is hereby extended one (1) year, to commence on October 14, 2016, and expire on October 13, 2017. Facilities Use Agreement P-50244 permits the Contractor exclusive use of the Premises on Saturdays and Sundays between 8:00AM and 5:00PM and weekday evenings as may be approved by the County in its reasonable discretion, for one (1) year with an option to renew by mutual consent. The Premises will be used to facilitate staff from the Contractor to provide a primary care clinic providing primary healthcare and primary mental health services to those experiencing homelessness. The Contractor will be responsible for complying with all federal, state and local laws, statutes, rules, regulations, ordinances codes and executive orders including Health Insurance Portability and Accountability Act of 1996 (HIPPA) compliance. The County will be responsible to provide Clinic Supplies and Medications. The Contractor will not be charged a license fee. AMENDMENT TO FACILITIES USE AGREEMENT BETWEEN ARIZONA BOARD OF REGENTS (ABOR) AND MARICOPA COUNTY Approve and authorize the Chairman to execute Amendment 3 to Facilities Use Agreement P-50244 between Arizona Board of Regents (ABOR), for and on behalf of (1) Arizona State University (ASU), (2) Northern Arizona University (NAU), and (3) University of Arizona (UA), (the “Contractor”) and Maricopa County for Contractor’s exclusive use of space (the “Premises”) within the County’s Health Care for the Homeless facility at the Human Services Campus at 220 S. 12th Avenue in Phoenix, Arizona. This amendment extends the Facility Use Agreement from October 14, 2017 through December 31, 2017. Facilities Use Agreement P-50244 permits the Contractor exclusive use of the Premises on Saturdays and Sundays between 8:00AM and 5:00PM and weekday evenings as may be approved by the County in its reasonable discretion, for approximately three and one-half (3½) months. The Premises will be used to facilitate Contractor staff to provide a primary care clinic healthcare and primary mental health services to those experiencing homelessness. 45. ASSIGNMENT AND ASSUMPTION OF FACILITIES USE AGREEMENT Approve and authorize the Chairman to execute Assignment and Assumption of Facilities Use Agreement, amendment No. 4 to P-50244. The Facilities Use Agreement P-50244 (FUA) is between the Arizona Board of Regents (“ABOR”), for and on behalf of: (1) Arizona State University (“ASU”); (2) Northern Arizona University (“NAU”); and (3) University of Arizona (“UA”), and Maricopa County for use of County property at 220 S. 12th Avenue to provide medical and mental health triage services, at no cost to presenting clientele. Maricopa County, as Assignor, will transfer all rights, obligations and interest in FUA P-50244 to the Human Services Campus, Inc., Assignee, (“HSC”) concurrent with the transfer of the property to HSC on or about October 4, 2017.
