39. MEMORANDUM OF UNDERSTANDING WITH MOUNTAIN PARK HEALTH CENTER FOR SUPPLEMENTAL NUTRITION ASSISTANCE EDUCATION Approve a Memorandum of Understanding (MOU) between Mountain Park Health Center and Maricopa County by and through its Department of Public Health (MCDPH) Nutrition and Physical Activity Division to collaborate alignment of nutrition education and obesity prevention services for SNAP eligible families. The terms of the Memorandum of Understanding (MOU) will be for three (3) years beginning October 1, 2014 through September 30, 2017, may be extended for two (2) years for a total of 5 years by mutual consent unless otherwise terminated by either party in writing with a 30 day notice. 49. AMENDMENT TO MEMORANDUM OF UNDERSTANDING WITH MOUNTAIN PARK HEALTH CENTER Approve a non-financial Amendment 1 to the Memorandum of Understanding (MOU) between the Mountain Park Health Center (MPHC) and Maricopa County by and through its Department of Public Health, Office of Community Health Innovations, for the purpose of continued collaboration on nutrition education and obesity prevention strategies for SNAP eligible or SNAP participating adults and children. The above named Memorandum of Understanding (MOU) is hereby amended as specified below. A. Extend the term from October 1, 2017 through September 30, 2019, at which time a new MOU would be required for continued partnership; B. Section C. Purpose: Remove reference to breastfeeding services and promotion efforts; C. Section D. Responsibilities: Remove 1. d. and 2. c.(references to breastfeeding services); All other terms and conditions of the original MOU shall remain in full force and effect. (C-86-15-031-3-01)
