50. FACILITY USE/REVOCABLE LICENSE AGREEMENT WITH KEOGH HEALTH CONNECTION FOR CLINICAL SERVICES SITE USE Approve and execute Facility Use/Revocable License Agreement with Keogh Health Connection (KHC) as user of County owned space located at 1645 E Roosevelt, Phoenix, AZ 85012 currently being used by Public Health/ Women, Infants and children (WIC) staff serving women, infants and children. KHC will provide assistance with enrollment into Department of Economic Services (DES), Arizona Health Care Cost Containment Systems (AHCCCS), Food Stamps and Cash Assistance services to eligible clients. This License Agreement is from October 22, 2014 to October 21, 2016 at no charge to the County or County clients. (C-86-15-032-M-00)

42. AMENDMENT TO FACILITIES USE/REVOCABLE LICENSE AGREEMENT WITH KEOGH HEALTH CONNECTION Approve and authorize the Chairman to execute Amendment No. 1 to Facility Use Revocable License Agreement P-50254, with Keogh Health Connection (KHC), an Arizona non-profit corporation, for continued use of space at 1645 E. Roosevelt Street, Phoenix, AZ. The Agreement will allow KHC to provide free on-site application assistance for enrollment into Department of Economic Services (DES) Arizona Health Care Cost Containment System (AHCCCS), SNAP and Cash Assistance, and Marketplace services to eligible clients. KHC will utilize space at the Roosevelt Public Health facility in assistance to WIC and Clinical Services programs on Monday, Thursday, and Friday, from 8 am to 5 pm. The amendment will become effective October 22, 2016 and expire October 31, 2018, extending the term approximately two years unless terminated earlier as provided for in the agreement. ADMINISTRATIVE CORRECTION TO FACILITY USE REVOCABLE LICENSE AGREEMENT WITH KEOGH HEALTH CONNECTIONS Correct the action taken on October 5, 2016, under C-86-15-032-M-01, regarding Facility Use Revocable License Agreement P-50254 between Maricopa County, Department of Public Health (MCDPH) and Keogh Health Connections. The Action Information Form originally stated “KHC will utilize space at the Roosevelt Public Health facility in assistance to WIC and Clinical Services programs on Monday, Thursday, and Friday, from 8 am to 5 pm”. This should be corrected to state: “KHC will utilize space at the Roosevelt Public Health facility in assistance to Clinical Services programs on Monday and Friday, from 8 am to 5 pm”. This is a non-financial agreement. The amendment is retro-active to October 22, 2016 and expires on October 31, 2018 unless terminated earlier as provided for in the agreement. There is no cost to either party. KHC provides a navigator to assist patients and visitors in exploring options and completing applications for enrollment to obtain health insurance coverage. 76. AMENDMENT TO FACILITIES USE/REVOCABLE LICENSE AGREEMENT WITH KEOGH HEALTH CONNECTION Approve and authorize the Chairman to execute Amendment No. 2 Facility Use Revocable License Agreement No. P-50254, with Keogh Health Connection (KHC), an Arizona non-profit corporation, for continued use of space at 1645 E. Roosevelt Street, Phoenix, AZ. The Agreement will allow KHC to continue to provide information on resources to prenatal families and parents of children 0-5 years old, Monday through Friday, between the hours of 8 AM and 5 PM, at the Roosevelt Public Health - WIC facility. The term of the Amendment will become effective November 1, 2018 and continue until terminated as provided for in the Agreement. Either Party may terminate the Agreement with a 30-day written notice to the other. The space will include one table and chairs. There is no cost to the County. (C-86-15-032-M-03)
