COMPETITION IMPRACTICAL CONTRACT WITH CENTRAL ARIZONA SHELTER SERVICES TO PROVIDE DENTAL SERVICES FOR THE HOMELESS Approve a Competition Impractical Contract between Central Arizona Shelter Services (CASS) and Maricopa County through its Department of Public Health, Health Care for the Homeless (HCH) and to provide dental services to homeless individuals. This Competition Impractical Contract is in compliance with the Maricopa County Procurement Code, Section MC1-351. This contract provides funds to CASS in the amount of $132,558 for the budget period November 1, 2014 to October 31, 2015 with the option of extending the contract for a total of five (5) years. Funding for this contract is provided by a grant from the Health Resources and Services Administration and will not affect the County General Fund. (C-86-15-045-3-00)

60. AMENDMENT TO COMPETITION IMPRACTICABLE CONTRACT WITH CENTRAL ARIZONA SHELTER SERVICES TO PROVIDE DENTAL SERVICES FOR THE HOMELESS Approve an amendment to the Competition Impracticable (CI) Contract between Central Arizona Shelter Services (CASS) and Maricopa County through its Department of Public Health, Health Care for the Homeless (HCH) and to provide dental services to homeless individuals. The CI contract is in compliance with the Maricopa County Procurement Code, Section MC1-351. This contract provides funds to CASS in the amount of $22,093.00 for the budget period November 1, 2015 to December 31, 2015 with the option of extending the contract for an additional five (5) years. 51. AMENDMENT TO COMPETITION IMPRACTICABLE CONTRACT WITH CENTRAL ARIZONA SHELTER SERVICES TO PROVIDE DENTAL SERVICES FOR THE HOMELESS Approve a retroactive Competition Impracticable (CI) Contract between Central Arizona Shelter Services (CASS) and Maricopa County through its Department of Public Health, Health Care for the Homeless (HCH) and to provide increased funds to CASS for dental services to homeless individuals within Maricopa County. The increased funds are $250,000.00 for the budget period November 1, 2015 to December 31, 2015. Also, the Contract amount is changed to an aggregate not-to-exceed total amount from $154,651.00 to $404,651.00. Additional amendments to the contract are: B. Coversheet, 6. Expiration Date, extend the end date to December 31, 2015. C. Section IV, COMPENSATION, 1. COMPENSATION, add the following paragraph: "D. CASS may charge HCH for expenses related to clinic staffing required to provide services as specified in this contract up to a maximum of $250,000." D. Section IV, COMPENSATION, 2. This CI amendment is in compliance with the Maricopa County Procurement Code, Section MC1-351. This amendment provides funds to CASS in the amount of $132,558.00 for the budget period January 1, 2016, through December 31, 2016. The contract includes options for the County to extend the contract for one-year periods for an additional four (4) years. Also, the contract’s aggregate not-to-exceed amount is changed from $404,651.00 to $537,209.00. AMENDMENT TO CONTRACT WITH BRIGHTER WAY DENTAL FOR DENTAL SERVICES Approve an Amendment No. 4 to the Competition Impractical Contract (CI) between Brighter Way Dental (fka Central Arizona Shelter Services (CASS) Dental Services) and Maricopa County through its Department of Public Health, Health Care for the Homeless (HCH) to provide dental services to homeless individuals. This CI contract is in compliance with the Maricopa County Procurement Code, Section MC1-351. This contract provides funds in the amount of $140,000.00 for the budget period January 1, 2017 through December 31, 2017, with an option of extending the contract for a maximum of five (5) additional years (through October 31, 2019). Also, the contract’s aggregate not-to-exceed amount is changed from $537,209.00 to $677,209.00 for the Contract period beginning November 1, 2014 through December 31, 2017. This action recognizes the Contractor name change from Central Arizona Shelter Services (CASS) Dental Services to Brighter Way Institute aka Brighter Way Dental. This name change applies to the Dental facility ONLY. Funding for this contract is provided by a grant from the Health Resources and Services Administration and will not affect the County General Fund. (C-86-15-045-3-04)
