34. AMENDMENT TO CONTRACT WITH DIGNITY HEALTH FOR COMMUNITY HEALTH NEEDS ASSESSMENT Approve an Amendment No. 1 to Contract for completion of a Community Health Needs Assessment between Dignity Health and Maricopa County by and through its Department of Public Health (MCDPH) to add 2 additional sites at $5,000 per site per year, in the amount of $30,000, with a three (3) year commitment beginning November 1, 2014 through October 31, 2017. This Contract may be extended for an additional three (3) years, not to exceed a total of six (6) years. In the past the Department of Public Health has provided such information to the health clinics and hospitals at no cost. AMENDMENT TO CONTRACT WITH DIGNITY HEALTH FOR COMMUNITY HEALTH NEEDS ASSESSMENT REPORT Approve an Amendment to the Contract for services between Dignity Health and Maricopa County by and through its Department of Public Health (MCDPH) in order to continue participating in the Community Health Needs Assessment (CHNA). The amendment is in the amount of $55,000 per year, for 3 years, for a total not-to-exceed amount of $165,000. The term of this Contract is revised to July 1, 2017 through June 30, 2020. This contract may be extended for additional three-year terms not to exceed a total extended term of 6 years; maximum extension through October 31, 2023. The Department of Public Health’s FY2018 indirect rate for fee fund is 12%; indirect costs are estimated at $17,679 all of which is fully recoverable. Previously, MCDPH would (at no charge) provide hospitals and Federally Qualified Health Centers (FQHCs) with extensive data requests and reports to support CHNA requirements. As requests for data became more extensive, MCDPH was unable to meet the demand while also meeting other Department obligations. (C-86-15-049-3-03)

Dignity Health is currently undergoing merger talks with another entity and will require a new contract. In an effort to preserve this agreement, Maricopa County Department of Public Health has agreed to reenter negotiations with Dignity Health (or the new entity). AMENDMENT TO CONTRACT WITH DIGNITY HEALTH FOR COMMUNITY HEALTH NEEDS ASSESSMENT REPORT Approve Amendment 3 to the Contract for services between Dignity Health and Maricopa County by and through its Department of Public Health (MCDPH) in order to continue participating in the Community Health Needs Assessment (CHNA). The amendment is in the amount of $55,000 for 1 year. The term of this Contract is revised to July 1, 2017 through June 30, 2018. This contract may be extended for additional three-year terms not to exceed a total extended term of 6 years; maximum extension through October 31, 2020. The Department of Public Health’s FY 2017-18 indirect rate for fee fund is 12.6%; indirect costs are estimated at $6,155 all of which is fully recoverable. Fee for service revenues are not local revenues for the purpose of the constitutional expenditure limitation, and therefore expenditure of the revenues is not prohibited by the budget law. This Agreement does not alter the budget constraining expenditures of local revenues duly adopted by the Board pursuant to A.R.S. §42-17105. The overall fee fund budget will be adjusted as necessary to accommodate these fees through a future reconciliation. OE will provide a CHNA to Dignity Health. The contract is amended as follows: A. Coversheet, 3. Contract Amount is changed to an aggregate, not-to-exceed (NTE) total amount from $220,000.00 to $290,000.00 (increased $70,000.00 for the budget period July 1, 2018 through June 30, 2019); B.
