52. RESCIND AND APPROVE AMENDMENT TO CONTRACT WITH BANNER HEALTH FOR COMMUNITY HEALTH NEEDS ASSESSMENT REPORT Rescind an action taken on April 12, 2017 for item (C-86-15-050-3-01) Amendment no. 1, Contract for services between Banner Health (BH), an Arizona nonprofit corporation and Maricopa County by and through its Department of Public Health (MCDPH) in order to continue participating in the Community Health Needs Assessment (CHNA). The amendment was in the amount of $300,000 and to extend the end date through June 30, 2020. BH requested to replace the Amendment no. 1 document with their Amendment no. 1 version with additional clauses and did not sign the original amendment. Also, approve the revised First Amendment to the Contract (0101-12-46149 A1) between Banner Health (BH), an Arizona nonprofit corporation and Maricopa County by and through its Department of Public Health (MCDPH) in order to continue participating in the Community Health Needs Assessment (CHNA). The amendment is in the amount of $100,000 per year, for 3 years, for a total not-to-exceed amount of $300,000. The term of this Contract is extended through June 30, 2020. This Amendment also includes Items 1. Amendment of the Contract; 2. Contract Effective; 3. Precedence of Amendment; 4. Capitalized Terms; 5. Incorporation; 6. Counterparts; and 7. Corporate Authority. The Department of Public Health’s FY2017-18 indirect rate for fee fund is 12.6%; indirect costs are estimated at $33,570 all of which is fully recoverable. Previously, the Department of Public Health would (at no charge) provide hospitals and Federally Qualified Health Centers (FQHCs) with extensive data requests and reports to support CHNA requirements. As requests for data became more extensive, PH was unable to meet the demand while also meeting other Department obligations. (C-86-15-050-3-02)
