49. AMENDMENT TO CONTRACT WITH AMERICAN LUNG ASSOCIATION IN ARIZONA FOR AZ SMOKE-FREE LIVING Approve Amendment No. 1 to Contract between the American Lung Association in Arizona (ALAA) and Maricopa County by and through its Department of Public Health (MCDPH), for the purchase of Smoke-Free Living Facilities services. The American Lung Association (ALA) in Arizona goals are to decrease exposure to secondhand smoke in homes by increasing access to smoke-free multi-unit housing option and to increase awareness among the public, healthcare community, and decision makers of the health risks and economic impact of poor air quality. The contract is amended as follows: A. 1.0. Contact Term, 1.1, change the contract term to one (1) month, beginning on the 1st day of October, 2015 and ending the 31st of October, 2015; B. 6.28 Incorporation of Documents, 6.28.1, Exhibit A, Pricing replace with an updated Exhibit A, Pricing, Budget Narrative for the budget period, October 1, 2015 – October 31, 2015 and budget amount of $53,185.00. All other terms and conditions of the original contract and its amendments shall remain in full force and effect. This Contractor is a successful respondent to a Request for Proposal PH RFP 15-006 issued by the Maricopa County Department of Public Health. Funding for this Contract is provided through an IGA between Maricopa County through its Department of Public Health, and Arizona Department of Health Services (ADHS) and does not increase the County general fund budget. All other terms and conditions of the original contract and its amendments shall remain in full force and effect. This Contractor is a successful respondent to a Request for Proposal PH RFP 15-006 issued by the Maricopa County Department of Public Health.
