20. IGA WITH ARIZONA DEPARTMENT OF HEALTH SERVICES FOR DIETETIC INTERNSHIP PROGRAM Approve an Intergovernmental Agreement (IGA) Contract No. ADHS15-084210 between Arizona Department of Health Services (ADHS) and Maricopa County by and through its Department of Public Health (MCDPH) to provide funding for the Dietetic Internship Program. The term of this Agreement is October 1, 2014 through September 30, 2019. The price sheet is effective October 1, 2014 through September 30, 2015 for the not-to-exceed amount of $150,000. The Department of Public Health’s Indirect Rate for FY15 is 14.8%. Full indirect costs are estimated at $19,338 of which is fully recoverable. Grant revenues are not local revenues for the purpose of the constitutional expenditure limitation, and therefore expenditure of the revenues is not prohibited by the budget law. This Amendment does not alter the budget constraining expenditures of local revenues duly adopted by the Board pursuant to A.R.S. §42-17105. The overall grant budget will be adjusted as necessary to accommodate this grant through a future reconciliation. Funding for this Agreement is provided by a Grant from ADHS and will not affect the County’s general fund. (C-86-15-061-3-00)

62. AMENDMENT TO IGA WITH ARIZONA DEPARTMENT OF HEALTH SERVICES FOR DIETETIC INTERNSHIP PROGRAM Approve a retroactive Amendment No. 1 to Intergovernmental Agreement (IGA) Contract No. (ADHS15-084210) between Arizona Department of Health Services (ADHS) and Maricopa County by and through its Department of Public Health (MCDPH) to provide funding for the Dietetic Internship Program. The term of this Agreement is October 1, 2014 through September 30, 2019. The revised price sheet is effective October 1, 2015 to September 30, 2016. Also, the total budget amount remains unchanged at $150,000.00, with the following changes: 1.1 Personnel Costs Salary increased by $2,400.00; 1.2 Employee Related Expense increased by $2,080.00; 1.3 Professional & Outside decreased by $265.00; 1.4 Travel Expenses decreased by $1,000.00 1.5 Occupancy Expenses increased by $4,000.00 1.6 Other Operating Expenses decreased by $4,343.00; 1.7 Capital Outlay remains the same; 1.8 Indirect increased by $1,128.00. The retroactive is due to the processing time at ADHS. All other provisions of this Agreement shall remain in their entirety. The Department of Public Health’s Indirect Rate for FY2015-16 is 15.8%. Full indirect costs are estimated at $20,466.00 of which is fully recoverable. Departmental indirect rates are reestablished at the beginning of each fiscal year and the future indirect rates will be collected at the corresponding rates. Grant revenues are not local revenues for the purpose of the constitutional expenditure limitation, and therefore expenditure of the revenues is not prohibited by the budget law. This Amendment does not alter the budget constraining expenditures of local revenues duly adopted by the Board pursuant to A.R.S. §42-17105. AMENDMENT TO IGA WITH ARIZONA DEPARTMENT OF HEALTH SERVICES FOR DIETETIC INTERNSHIP PROGRAM Approve Amendment No. 2 to Intergovernmental Agreement (IGA) Contract No. (ADHS15-084210) between Arizona Department of Health Services (ADHS) and Maricopa County by and through its Department of Public Health (MCDPH) to provide funding for the Dietetic Internship Program. The term of this Agreement is October 1, 2014 through September 30, 2019. The revised price sheet is effective October 1, 2016 to September 30, 2017. Amendment Two (2) include the following changes: 1. Pursuant to the Terms and conditions, Provision Twenty (20), The Federal Funding Accountability and Transparency Act (FFATA or Transparency Act – P.L. 109-282, as amended by Section 6202 (A) of P.L. 110-252) 2. The Dietetic Internship Price Sheet in Amendment One (1) is hereby amendment with the revised Dietetic Internship Price Sheet of this Amendment Two (2). The total of the Dietetic Internship Price Sheet is $150,000.00. Section 2., will include the following changes: 2.1 Personnel Costs Salary decreased by $288.00; 2.2 Employee Related Expense increased by $1,997.00; 2.3 Professional & Outside increased by $2,580.00; 2.4 Travel Expenses decreased by $823.00 2.5 Occupancy Expenses increased by $356.00 2.6 Other Operating Expenses decreased by $3,823.00; 2.7 Capital Outlay Expenses increased by a $1.00; 2.8 Indirect Cost Expenses will remain in the same dollar amount of $20,466.00. 3. In ProcureAZ the “items” Tab of the Master Blanket Purchase Order will be revised to reflect the pricing upon execution of this Amendment Two (2). All Other Provisions of this agreement shall remain in their entirety. The Department of Public Health’s Indirect Rate for FY2016-17 is 15.1%. Full indirect costs are estimated at $19,679.00 of which is fully recoverable. Departmental indirect rates are reestablished at the beginning of each fiscal year and the future indirect rates will be collected at the corresponding rates. Grant revenues are not local revenues for the purpose of the constitutional expenditure limitation, and therefore expenditure of the revenues is not prohibited by the budget law. AMENDMENT TO IGA WITH ARIZONA DEPARTMENT OF HEALTH SERVICES FOR DIETETIC INTERNSHIP PROGRAM Approve Amendment No. 4 to Intergovernmental Agreement (IGA) Contract No. (ADHS15-084210) between Arizona Department of Health Services (ADHS) and Maricopa County by and through its Department of Public Health (MCDPH) to provide continued funding for the Dietetic Internship Program. The term of this Agreement is October 1, 2014 through September 30, 2019. The revised price sheet is effective October 1, 2018 to September 30, 2019 and includes the following changes: 1. The Dietetic Internship Price Sheet in Amendment 3 is hereby amended with the revised Dietetic Internship Price Sheet of this Amendment 4. The total budget amount remains $150,000.00. All Other Provisions of this agreement shall remain in their entirety. This grant award is reoccurring and has been awarded to the department for many years. There is no cash or in-kind match required and indirect costs are fully recoverable. Ongoing cash contributions will not be required. The grant award is not competitively bid. The department’s operating budget is not required to absorb any costs. The Department of Public Health’s Indirect Rate for FY2018-19 is 18.9%. Indirect costs are budgeted at $23,844.00 which are fully recoverable. Departmental indirect rates are reestablished at the beginning of each fiscal year and the future indirect rates will be collected at the corresponding rates. Grant revenues are not local revenues for the purpose of the constitutional expenditure limitation, and therefore expenditure of the revenues is not prohibited by the budget law. This Amendment does not alter the budget constraining expenditures of local revenues duly adopted by the Board pursuant to A.R.S. §42-17105. The overall grant budget will be adjusted as necessary to accommodate this grant through a future reconciliation.
