72. AGREEMENT WITH UNITED HEALTHCARE INSURANCE COMPANY FOR PROVIDER HEALTH CARE SERVICES Approve a Participation Health Provider Agreement between United Healthcare Insurance Company, contracting on behalf of itself Arizona Physicians IPA, Inc., and other entities that are United’s Affiliates, Collectively referred to as “United” and Maricopa County by and through its Department of Public Health, Maricopa County, dba Maricopa County Department of Public Health, dba Public Health Clinical Services to provide certain covered medical and related services to eligible” United” Plan Members. This Agreement is a fee-for-service agreement. This agreement shall become effective upon signatures of both parties and shall continue for three (3) years and renew automatically for renewal terms of one year, unless terminated by either party with a one-hundred twenty (120) day written notice. This Agreement allows Maricopa County by and through its Department of Public Health, Maricopa County, dba Maricopa County Department of Public Health, dba Public Health Clinical Services to bill “United” for reimbursable client services provided at the Public Health Clinics which includes Health Care for the Homeless Clinic, a federally grant funded program, to generate additional revenue for the program. This agreement is part of Healthcare for the Homeless grant agreement as program income. (C-86-15-079-3-00)
