77. MOU WITH ARIZONA DEPARTMENT OF HEALTH SERVICES AND MARICOPA INTEGRATED HEALTH SYSTEM Approve a Memorandum of Understanding (MOU) between Arizona Department of Health Services (ADHS), Maricopa Integrated Health System (MIHS) and Maricopa County by and through its Department of Public Health (MCDPH). This Agreement allows the parties to develop plans and share their collective health expertise and resources to enable MIHS to meet the needs of the patients. The terms of the Memorandum of Understanding (MOU) will be for one (1) year beginning on March 11, 2015 through March 10, 2016, may be extended for two (2) years for a total of three (3) years by mutual consent unless otherwise terminated by either party in writing with a 30 day notice. 63. AMENDMENT TO MOU WITH ARIZONA DEPARTMENT OF HEALTH SERVICES AND MARICOPA INTEGRATED HEALTH SYSTEM FOR RESOURCE SHARING Approve an Amendment No. 1 to Memorandum of Understanding (MOU) between Arizona Department of Health Services (ADHS), Maricopa Integrated Health System (MIHS) and Maricopa County by and through its Department of Public Health (MCDPH). This Agreement allows the parties to develop plans and share their collective health expertise and resources to enable MIHS to meet the needs of the patients. Pursuant to the Terms and Conditions, Section Eight (8), Contract Extension, the term of the agreement is hereby extended to April 28, 2018 by this Amendment One (1). Also, Section Six (6), 6.2.1., Insurance and Indemnification will be replaced. All other provisions shall remain in their entirety. (C-86-15-082-M-01)
