64. RFP CONTRACT WITH BIO-REFERENCE LABORATORIES, INC FOR LABORATORY TESTING SERVICES Approve Request for Proposal (RFP) Contract (PH RFP 15-008) between Bio-Reference Laboratories (BRL), Inc. and Maricopa County by and through its Department of Public Health, Clinical Services to provide laboratory testing services. The term of the contract is for three (3) years starting April 8, 2015 ending April 7, 2018. The cost of the services are not expected to exceed $200,000 per year for a total of $600,000. The County may, at its option and with the agreement of the Contractor, renew the term of this Contract for additional terms up to a maximum of five (5) years, (or at the County’s sole discretion, extend the contract on a month-to-month basis for a maximum of six (6) months after expiration). BRL is a successful respondent to a Request for Proposal (PH RFP 15-008) issued by MCDPH pursuant to MC1-328. (C-86-15-086-3-00)

52. AMENDMENT TO CONTRACT WITH BIO-REFERENCE LABORATORIES Approve Amendment No. 1 to Request for Proposal (RFP) Contract 860-15-08B between Bio-Reference Laboratories and Maricopa County through its Department of Public Health, Clinical Services, to provide Laboratory Testing Services. This Amendment extends the contract on a month-to-month basis for three (3) months after expiration, beginning on April 8, 2018, and ending July 7, 2018. County may extend for an additional 3 months. The Contract extension amount is not to exceed $16,600 per month for each of the 3 months for a not to exceed amount of $49,800. All other terms and conditions of the original contract shall remain in full force and effect. Contractor is a successful respondent to a Request for Proposal (PH RFP 15-008) and procured under Section MC1-328 of the Maricopa County Procurement Code Competitive Sealed Proposals.
