39. MOU WITH ARIZONA DEPARTMENT OF HEALTH SERVICES FOR SHARING OF VITAL RECORDS DATA Approve a Memorandum of Understanding (MOU) No. HU456045 between Arizona Department of Health Services (ADHS), and Maricopa County by and through its Department of Public Health (MCDPH). This Agreement allows the parties to share vital records data. The terms of the MOU will be for five (5) years commencing upon signature of both parties unless otherwise terminated by either party in writing with a 30 day notice. (C-86-15-089-M-00)
