36. AMENDMENT TO CONTRACT WITH THE ARIZONA SMALL BUSINESS ASSOCIATION FOR WORKSITE WELLNESS INITIATIVE SERVICES Approve an Amendment to Contract 860-12-033 (PH RFP 12-033) between the Arizona Small Business Association (ASBA) and Maricopa County by and through its Department of Public Health for the purchase of Worksite Wellness Initiative Services. This Amendment adds $30,000 to the budget for a not-to-exceed total of $511,735. The term of this contract is through June 30, 2015. All other terms and conditions of the original contract and its amendments shall remain in full force and effect. The original RFP 12-033 (Contract 860-12-033) was approved by Materials Management and Amendments No. 1 – 6. This Amendment puts the dollar amount over the threshold of Materials Management limit of ($500,000) and therefore needs to be approved by the Board of Supervisors. (C-86-15-090-3-00)
