41. AMENDMENT TO RFP CONTRACT WITH THE ARIZONA PARTNERSHIP FOR IMMUNIZATION Approve Amendment No. 2 to Request For Proposal (RFP) Contract (860-14-020) between The Arizona Partnership for Immunization (TAPI) and Maricopa County by and through its Department of Public Health (MCDPH) to provide additional billing research services. The research services include cost/benefit analysis of third-party reimbursement for Chronic Disease (Exhibit B1) and HIV (Exhibit B2) Services. Also, added is a Business Associate Agreement (Exhibit C). The RFP contract term is for a maximum of fine (5) years, (or at the County’s sole discretion, extend the contract on a month-to-month basis for a maximum of six (6) months after expiration). The term is from March 4, 2014 through March 3, 2019. The RFP Contract will be amended as follows: A. Cover Page, line 6, of the original contract shall be deleted and replaced with the following: “…the 30th day of December, 2015.” B. Exhibit A, Total Amount on the Pricing/Compensation page of the original contract, shall be changed to $168,269.12 which is an increase of $68,269.12. C. Exhibit B of the original contract shall be deleted and replaced with the newly added Exhibits B-1 and B-2 beginning on Page 3 of this amendment. D. $25,000 of the $68,269.12 increase shall be used for the deliverables as shown on the new Exhibit B-1. E. $43,269.12 of the $68,269.12 increase shall be used for the deliverables as shown on the new Exhibit B-2. F. Under 6.25, Incorporation of Documents, 6.25.3., Exhibit C, Business Associate Agreement shall be added. The base Public Health (PH) RFP Contract 14-020 (860-14-020) was entered into on March 4th, 2014 and approved under the authorized signature limit of the Maricopa County Office of Procurement Services Chief Procurement Officer. Amendment No. 1, to PH RFP Contract 14-020 (860-14-020) was processed on June 22, 2014. (C-86-15-093-3-00)
