56. PARTNERSHIP AGREEMENT WITH CREIGHTON ELEMENTARY SCHOOL DISTRICT FOR NUTRITIONAL AND PHYSICIAL ACTIVITY EDUCATION Approve a non-financial Partnership Agreement (PA) between Creighton Elementary School District (CESD) and Maricopa County by and through its Department of Public Health (MCDPH), Arizona Nutrition Network (AzNN) to provide nutrition and physical activity education programs and services to the schools in this district. This PA is part of the AzNN Grant. This PA will be in effect from October 1, 2015 through September 30, 2018. (C-86-15-099-M-00)
