AFFILIATION AGREEMENT WITH QUEEN CREEK UNIFIED SCHOOL DISTRICT # 95 FOR DIETETIC INTERNSHIP PROGRAM Approve the Affiliation Agreement entitled “Dietetic Internship Program” with Queen Creek Unified School District #95 (QCUSD) and Maricopa County by and through its Department of Public Health (MCDPH) to provide clinical nutrition training experience for graduate students in the dietetic internship program. This Agreement is non-financial, and the term is to be from July 1, 2015 through June 30, 2020. (C-86-15-108-M-00)
