57. AMENDMENT TO REVOCABLE NON-EXCLUSIVE LICENSE AGREEMENT P-50277 WITH THE HELPING FAMILIES IN NEED Approve and execute Amendment No. 3 to Revocable Non-Exclusive License Agreement P-50277, “Agreement” between Maricopa County, “County” and The Helping Families in Need (“HFIN”) an Arizona non-profit corporation “Contractor” for use of the County Premises located at 635 E Broadway, Mesa, AZ 85204 and 1850 N, 95th Ave., Suite 184, Phoenix, AZ 85307. This Amendment will extend the term of the Agreement for two (2) years commencing November 1, 2021 and expire October 31, 2023 unless terminated earlier as provided for in the Agreement. Either party may terminate the agreement with a thirty (30) day written notice to the other or by mutual written agreement. There is no cost to the County or to presenting clientele. This Agreement will allow HFIN to provide on-site application assistance for enrollment into Department of Economics Services (“DES”), Arizona Heal Care Cost Containment System (“AHCCCS”), Supplemental Nutrition Assistance Program (“SNAP”), Cash Assistance and Marketplace services to all presenting Women, Infants and Children (“WIC”) eligible clients. This action takes place in Supervisorial District No’s. 2 & 5 (C-86-16-024-M-03)
