54. COMPETITION IMPRACTICABLE CONTRACT WITH HUMAN SERVICES CAMPUS, LLC FOR MEDICAL ASSESSMENT Approve a retroactive Fixed-Price Competition Impracticable (CI) Contract between Human Services Campus (HSC), LLC and Maricopa County through its Department of Public Health, Health Care for the Homeless (HCH) and to provide funds to HSC for developing and implementing a program for medical assessments to homeless individuals that qualify for permanent housing assistance within Maricopa County. The budget amount is $150,000.00 for the budget period December 1, 2015 through November 30, 2016. Subject to the availability of funds and acceptable Contractor performance, the Contractor hereby acknowledges and agrees that the County shall have the right to extend this Contract for additional one-year periods, not to exceed a total extended term of five (5) years, except that the cost will be subject to renegotiation. The retroactive is due to the processing time with the Contractor. Funding for this contract is provided by a grant from the Health Resources and Services Administration and will not affect the County General Fund. The CI contract is in compliance with the Maricopa County Procurement Code, Section MC1-351. (C-86-16-046-3-00)

44. AMENDMENT TO COMPETITION IMPRACTICABLE CONTRACT WITH HUMAN SERVICES CAMPUS, LLC, FOR MEDICAL ASSESSMENT Approve a retroactive amendment to the Fixed-Price Competition Impracticable (CI) Contract between Human Services Campus (HSC), LLC and Maricopa County through its Department of Public Health, Health Care for the Homeless (HCH) to continue to provide funds to HSC for developing and implementing a program for medical assessments to homeless individuals that qualify for permanent housing assistance within Maricopa County. The budget will be increased by $75,000 for the budget period December 1, 2016 through December 31, 2017, bringing the total not-to-exceed contract amount from $150,000 to $225,000. All other terms of this agreement remain unchanged. Funding for this contract is provided by a grant from the Health Resources and Services Administration and will not affect the County General Fund. The CI contract is in compliance with the Maricopa County Procurement Code, Section MC1-351. (C-86-16-046-3-01)
