36. FACILITY USE REVOCABLE LICENSE AGREEMENT WITH WESLEY COMMUNITY CENTER, INC. FOR OFFICE SPACE Approve, and authorize the Chairman to execute, Facility Use Revocable License (FURL) Agreement No. P-50283, with Wesley Community Center, Inc., (WCC) for use of space at two (2) Maricopa County through its Department of Public Health, Women, Infant and Children (WIC) clinics. The agreement also permits the Public Health Director, or their designee, to change the location, date or time of USE and parking, as long as there is no cost to the County, at the sole discretion of the Director. The Agreement will allow WCC to provide free on-site application assistance for enrollment into Department of Economic Services (DES), Arizona Health Care Cost Containment System (AHCCCS), Supplemental Nutrition Assistance Program (SNAP), Cash Assistance, and Marketplace service to all Women, Infants and Children (WIC) eligible clients at the Public Health/WIC facility. The term of the agreement will begin February 1, 2016 and expire January 31, 2018, unless terminated earlier as provided for in the agreement. There is no cost to the County or presenting clientele. Either Party may terminate the Agreement with 30 days’ notice to the other Party. The two WIC facility locations are: 1) South Phoenix WIC Clinic, 438 E. Southern Avenue, Phoenix, AZ 85040 2) Seventh Avenue WIC Clinic, 1260 S. 7th Avenue, Phoenix, AZ 85007 (C-86-16-051-3-00)

66. RESCIND ACTION REGARDING FACILITY USE REVOCABLE LICENSE AGREEMENT BETWEEN WESLEY COMMUNITY CENTER, INC. AND MARICOPA COUNTY Rescind agenda item C-86-16-051-3-00 that approved and authorized the Chairman to execute Facilities Use / Revocable License Agreement No. P-50283, with Wesley Community Center (WCC), Inc., an Arizona non-profit corporation. Due to lack of capacity, Wesley Community Center regretfully requests that the County rescind the Agreement approved at the 01/06/2016 Board of Supervisors Meeting. This Agreement was to allow WCC to provide on-site enrollment assistance to program eligible clients. Due to current staff demands at WCC, it is felt that they would be unable to adequately serve WIC clientele, while continuing to successfully fulfill other Wesley Community Center obligations. The two WIC facility locations were to be: 1)South Phoenix WIC Clinic, 438 E. Southern Avenue, Phoenix, AZ 85040 2)Seventh Avenue WIC Clinic, 1260 S. 7th Avenue, Phoenix, AZ 85007 (C-86-16-051-3-01)
