75. IGA WITH ARIZONA DEPARTMENT OF HEALTH SERVICES FOR THIRD PARTY REIMBURSEMENT FOR FLOURIDE VARNISH SERVICES Approve retroactive fixed-price Intergovernmental Agreement (IGA), between Maricopa County by and through its Department of Public Health (MCDPH) Office of Oral Health (OOH) and Arizona Department of Health Services (ADHS) “Contractor”. ADHS shall provide services related to billing and reimbursement for fluoride varnish services rendered through the First Teeth First program. The fixed-price budget total amount is not-to-exceed $108,264.00 for year one (1). The fixed amount payable each month is $9,022.00 for the budget period July 1, 2015, through June 30, 2016. Subject to the availability of funds and acceptable Contractor performance, the Contractor hereby acknowledges and agrees that the County shall have the right to extend this Agreement for additional one-year periods, not to exceed a total extended term of five (5) years, except that the cost will be subject to renegotiation. Any extension of this Agreement shall be in writing mutually acceptable to the MCDPH and the Contractor and signed by both Parties. (C-86-16-080-3-00)

AMENDMENT TO IGA WITH ARIZONA DEPARTMENT OF HEALTH SERVICES Approve a retro-active Amendment 1 to the fixed-price Intergovernmental Agreement (IGA) between Maricopa County by and through its Department of Public Health (MCDPH) and The Arizona Department of Health Services (ADHS). This contract will continue the program whereby ADHS provides services related to billing and reimbursement for fluoride varnish services rendered through the First Teeth First program. MCDPH will receive payment for appropriately billed services (administration fees and/or billing and reimbursement fees) for services appropriately billed to privately insured clients as well as clients listed to be on the Arizona State Medicaid Program, Arizona Health Care Cost Containment System (AHCCCS). Generated revenue for MCDPH is estimated not to exceed $200,000.00 for the budget period July 1, 2015 through June 30, 2016. (C-86-16-080-3-02)

AMENDMENT TO IGA WITH ARIZONA DEPARTMENT OF HEALTH SERVICES FOR FIRST TEETH FIRST PROGRAM Approve Amendment 2 to the fixed-price IGA between Maricopa County by and through its Department of Public Health (MCDPH) and The Arizona Department of Health Services (ADHS). This contract will continue the program whereby ADHS provides services related to billing and reimbursement for fluoride varnish services rendered through the First Teeth First program. The fixed-price budget total remains the not to exceed amount of $108,264.00 ($9,022.00 payable each month) for year two (2). MCDPH will continue to receive the revenue for appropriately billed services (administration fees and/or billing and reimbursement fees) for services billed to privately insured clients as well as clients listed to be on the Arizona State Medicaid Program, Arizona Health Care Cost Containment System (AHCCCS). AMENDMENT TO IGA WITH ARIZONA DEPARTMENT OF HEALTH SERVICES FOR THIRD PARTY REIMBURSEMENT SERVICES Approve Amendment 3 to the fixed-price Intergovernmental Agreement (IGA) between Maricopa County by and through its Department of Public Health (MCDPH) and The Arizona Department of Health Services (ADHS). This contract will continue the program whereby ADHS provides services related to billing and reimbursement for fluoride varnish services rendered through the First Teeth First program. The fixed-price budget total is $108,264 for the budget period of July 1, 2017 through June 30, 2018. And the total budget not to exceed amount to $216,528 for budget term is July 1, 2015 through June 30, 2018. MCDPH will continue to receive the revenue for appropriately billed services (administration fees and/or billing and reimbursement fees) for services billed to privately insured clients as well as clients listed to be on the Arizona State Medicaid Program, Arizona Health Care Cost Containment System (AHCCCS).
