53. FACILITIES USE REVOCABLE LICENSE AGREEMENT WITH CHANDLER UNIFIED SCHOOL DISTRICT NO. 80 Approve and authorize the Chairman to execute Facilities Use Revocable License Agreement (FURL) P-50289, with Chandler Unified School District No. 80 (CUSD), for use of space at one (1) Maricopa County Department of Public Health Women, Infant and Children (MCDPH-WIC) Clinic for the summer meals program. The agreement term will begin June 1, 2016, and expire July 15, 2016, unless terminated earlier as provided for in the agreement. Either Party may terminate the agreement with 30 days’ notice to the other. Article 4.3 of the agreement allows the MCDPH director, or his designee, to change the date, time or location of the use so long as there is no cost to the County. The FURL will allow CUSD to provide a Summer Meal Service Program to youth under age 18 at the MCDPH-WIC facilities. The WIC facility is located at 3002 N. Arizona Avenue, Suite 13, Chandler, AZ 85225. (C-86-16-096-3-00)

56. AMENDMENT TO FACILITIES USE REVOCABLE LICENSE AGREEMENT WITH CHANDLER UNIFIED SCHOOL DISTRICT NO. 80 Approve and authorize the Chairman to execute Amendment 1 to the Facilities Use Revocable License Agreement (FURL) P-50289, with Chandler Unified School District No. 80 (CUSD), for use of space at one (1) Maricopa County Department of Public Health Women, Infant and Children (MCDPH-WIC) Clinic for the summer meals program. Amend the following provision: Article 5. Term and Termination of Agreement 5.1 The term of this agreement shall commence on June 1, 2016 and shall expire July 15, 2016 (the “term”). The FURL allows CUSD to provide a Summer Meal Service Program to youth under age 18 at the MCDPH-WIC facilities. The WIC facility is located at 3002 N. Arizona Avenue, Suite 13, Chandler, AZ 85225.
