50. LEASE AGREEMENT WITH BOEHLE PROPERTIES, LLC FOR VITAL REGISTRATION FACILITY Approve, and authorize the Chairman to execute Lease Agreement L-7462 with Boehle Properties, LLC, an Arizona limited liability company (Lessor), for a Vital Registration satellite location at 2423 West Dunlap Avenue, Suite 110, Phoenix, Arizona, (the “Premises”). This new lease is for 2,865 rentable square feet of office and clinic space within Suite 110. The lease allows the Department of Public Health to operate a Vital Registration facility for a lease term of sixty two (62) months (“term”) commencing upon completion of Lessor Tenant Improvements, and expire after 62 months. The County will pay base rent of $4,894.38 per month during the first lease year with a 2.3% per year escalation thereafter. The County will reimburse Lessor an amount not to exceed up to $160,000.00 for Tenant Improvements. Lessor shall contribute $35,000.00 toward Tenant Improvements.
