40. PARTNERSHIP AGREEMENT WITH ADELANTE HEALTHCARE FOR ORAL HEALTH SCREENINGS Approve a non-monetary Partnership Agreement between Maricopa County, by and through its Department of Public Health, Office of Oral Health, and Adelante Healthcare, through its WIC program, to provide oral health screenings and education, as well as fluoride varnish applications to children 0-5 and pregnant women. (C-86-17-006-3-00) 41. RESCIND ACTION AND APPROVE REVISED AMENDMENT TO LEASE AGREEMENT WITH HTA-THUNDERBIRD MEDICAL, LLC Rescind action taken by the Board on July 20, 2016 under agenda item C-86-05-566-4-03 and approve and execute a revised Amendment No. 2 to Lease Agreement No. L-7357 with HTA-Thunderbird Medical, LLC, Lessor, for 4,292 square feet of office/clinic space located at 5422 W. Thunderbird Road, Glendale, Arizona. The County WIC is occupying the leased premises via the “hold-over” provision of the original agreement for the months of August and September of 2016. Amendment No. 2, as revised, extends the term of the lease for an additional four (4) years and four (4) months to commence October 1, 2016 and expire January 31, 2021. The initial lease rate will be reduced to $20.00 per square foot and will increase 3% annually with one (1) month gratis for months 12, 24, 36, and 48. The County may terminate the Lease at the end of any fiscal year for non-availability of funds. (C-86-05-566-4-04)
