PARTNERSHIP AGREEMENT WITH DIGNITY HEALTH MEDICAL GROUP FOR ORAL HEALTH SCREENINGS Approve a retro-active, non-monetary Partnership Agreement (PA) between Maricopa County, by and through its Department of Public Health, Office of Oral Health, and Dignity Health dba Dignity Health Medical Group for Maternity Outreach Mobile (MOM), to provide oral health screenings and education, as well as fluoride varnish applications to children 0-5 and pregnant women.
