AMENDMENT TO PUBLIC USE DATA AGREEMENT WITH CITY OF MESA THROUGH ITS PUBLIC HOUSING AUTHORITY (MHA) FOR CHILDHOOD LEAD POISONING DATA Approve an amendment to the non-financial Public Use Data Agreement (PUDA), between City of Mesa through its Public Housing Authority (MHA) and Maricopa County by and through its Department of Public Health Services (MCDPH), Office of Family Health, to facilitate a childhood lead poisoning data gathering project. This agreement is renewed beginning December 1, 2019 and continues until terminated with 30 day notice by either party. This PUDA is in response to (C-86-17-032-3-02), Memorandum of Understanding (MOU), HU756005, Amendment No. 2, Exhibit P – Childhood Lead Poisoning Data. MOU HU756005 is in response to U.S. Department of Housing and Urban Development, Lead-Safe Housing Rule (24 CFR 35, Subpart L). (C-86-19-032-3-01)
