AMENDMENT TO IGA WITH PEORIA UNIFIED SCHOOL DISTRICT FOR SCHOOL-BASED TOBACCO USE PREVENTION AND EDUCATION SERVICES - STAND Approve amendment # 4 to the Intergovernmental Agreement (IGA) between Peoria Unified School District and Maricopa County by and through its Department of Public Health (MCDPH) to continue to provide school-based Tobacco Use Prevention and Education Services; Students Taking A New Direction (STAND). Funding is in the not-to-exceed amount of $2,500 for the budget period beginning July 1, 2020 through June 30, 2021 for a total not-to-exceed amount of $9,000.00 for the budget term August 1, 2016 through June 30, 2021. All other terms and conditions of the original contract and its amendments shall remain in full force and effect. (C-86-17-041-3-04)

48. AMENDMENT TO IGA WITH PEORIA UNIFIED SCHOOL DISTRICT FOR SCHOOL-BASED TOBACCO USE PREVENTION AND EDUCATION SERVICES - STAND Approve amendment 5 to the Intergovernmental Agreement (IGA) between Peoria Unified School District and Maricopa County by and through its Department of Public Health (MCDPH) to continue to provide School-based Tobacco Use Prevention and Education Services; Students Taking A New Direction (STAND). An amendment is needed to add changes to the school site coordinator and coalition requirements as a result of COVID-19. The revised attachment “B”, is effective August 1, 2020 to June 30, 2021 and includes the following changes: I. Per Section III, 4. of the above named IGA, this contract is hereby amended as specified below: A. ATTACHMENT B is revised II. ADMINISTRATIVE CORRECTION TO IGA AMENDMENT BETWEEN DEPARTMENT OF PUBLIC HEALTH AND PEORIA UNIFIED SCHOOL DISTRICT STAND PROGRAM Approve an administrative correction to the action taken on October 21, 2020, under item C-86-17-041-3-05 to Amendment 5 of the Intergovernmental Agreement (IGA) between Peoria Unified School District and Maricopa County by and through its Department of Public Health (MCDPH) to continue to provide School-based Tobacco Use Prevention and Education Services; Students Taking A New Direction (STAND). The administrative correction is needed to reflect changes to “Attachment B” of the school site coordinator requirements to remove previously added COVID related coalition activities and update stipend requirements.
