56. AMENDMENT TO IMPLEME NTATION AGREEMENT WITH NURSE FAMILY PARTNERSHIP FOR THE NURSE-FAMILY PARTNERSHIP PROGRAM Approve an amendment to Implementation Agreement (IA) between Nurse Family Partnership (NFP), a Colorado Nonprofit Corporation and Maricopa County by and through its Department of Public Health (MCDPH), Office of Family Health (OFH) for additional funding in the amount of $95,000 for the implementation of the evidence-based Nurse Family Partnership® Program. The IA term is November 1, 2018 through October 31, 2021resulting in a total of $199,145 for the contract term. Automatic extensions apply until term inated by either party. Fees for NFP training/education services will be billable and paid through grant funding from Arizona Department of Health Services. The grant award for the Nurse-Family Partnership Home Visitation in South Phoenix was approved by the Board on 5/23/2018, as C-86-17-049-3-02. Procurement authority is Prior Designation – MC1-102C. The Nurse Family Partnership Program is an evidence based model program for low income, first time mothers and their children. This Agreement specifies the mechanisms and means for continued implementation of the Program and NFP’s training and support services. This Agreement allows the Office of Family Health to use the NFP model in its provision of community health nursing services. MCDPH, Office of Fam ily Health, has adopted and implemented the Nurse Family Partnership model in the South Phoenix Region to provide comprehensive community health nursing services to eligible women and their families. Through this program, MCDPH has established and expanded home visitation programs to support parents and caregivers in the ability to promote their children’s optimal development and health. 86. AMENDMENT TO ADHS IGA FOR THE NURSE-FAMILY PARTNERSHIP MIECHV PROGRAM Approve Amendment 1 to Inter-Governmental Agreement (IGA) Contract No ADHS17-155910, Nurse Family Partnership (NFP) for Maternal, Infant, and Early Childhood Home Visiting (MIECHV), between the Arizona Department of Health Services (ADHS) and Maricopa County through its Department of Public Health (MCDPH) to provide additional funding for the NFP MIECHV program. The Price Sheet is in the amount of $1,631,523 for the budget period October 1, 2020 through September 30, 2021. The Contract term is May 1, 2017 through April 30, 2022. The NFP MIECHV IGA award is reoccurring and has been awarded to the department for many years. This grant does not require an in-kind match, indirect cost is fully recoverable and ongoing cash contributions are not required. The IGA award is not a mandated function but provides a benefit to the citizens of Maricopa County by providing evidence-based home visiting models to communities identified to be at risk. The award is competitive. The Department of Public Health FY 2020-21 indirect rate is 18.8%. The pass thru amount in the anticipated amount of $499,231 to sub-recipient Southwest Human Development is not subject to indirect. Full indirect costs are estimated at $179,184 all of which is recoverable. Program costs not covered by the grant will be subsidized by the MCDPH indirect cost pool. Departmental indirect rates are re-established at the beginning of each fiscal year and the future indirect rates will be collected at the corresponding rates. Grant revenues are not local revenues for the purpose of the constitutional expenditure limitation, and therefore expenditure of the revenues is not prohibited by the budget law. This Amendment does not alter the budget constraining expenditures of local revenues duly adopted by the Board pursuant to A.R.S. 42-17105. The overall grant budget will be adjusted as necessary to accommodate this grant through a future reconciliation. Funding for this grant will be provided by ADHS and will not affect the County general fund. (C-86-17-049-3-07)

PURCHASE ORDER FOR IGA WITH ADHS FOR THE NURSE-FAMILY PARTNERSHIP FOR MIECHV, HOME VISITATION PROGRAM Approve Purchase Order (PO) PO371937 for Inter-Governmental Agreement (IGA) Contract No. (ADHS17-155910, Nurse Family Partnership (NFP) for MIECHV, between the Arizona Department of Health Services (ADHS) and Maricopa County through its Department of Public Health (MCDPH) to provide additional funding for the NFP, Maternal, Infant, and Early Childhood Home Visiting (MIECHV). The additional amount is $815,761.50, bringing the total not to exceed amount from $1,631,523 to $2,447,284.50, for the IGA term of October 1, 2017 through March 31, 2022. The Contract term is modified to read May 1, 2017 through March 31, 2022. The NFP MIECHV IGA award is reoccurring and has been awarded to the department for many years. This grant does not require an in-kind match, indirect cost is fully recoverable and ongoing cash contributions are not required. The IGA award is not a mandated function but provides a benefit to the citizens of Maricopa County by providing evidence-based home visiting models to communities identified to be at risk.
