AMENDMENT TO REVOCABLE LICENSE AGREEMENT WITH DEER VALLEY UNIFIED SCHOOL DISTRICT Approve Amendment No. 2 to License Agreement No. P-50298 between Deer Valley Unified School District (DVUSD), Licensor, and Maricopa County, Licensee, for use of two classrooms at DVUSDs’ Constitution Elementary School located at 18440 North 15th Avenue, Phoenix, AZ 85023. Maricopa County Public Health, through its Women Infants and Children (WIC) Program will provide continuing WIC program services at no charge to presenting clientele including services from associated partners. The extended term under the amended License will commence October 1, 2020 and expire September 30, 2022. The monthly use fee will continue at nine hundred ($900.00) dollars (gross) for 24 months for a total not-to-exceed of $21,600 plus 10% tax and/or miscellaneous; an additional $2160.00.
