VINCENT DE PAUL FOR ADULT IMMUNIZATION SERVICESApprove and authorize the Chairman to execute a MOU between Maricopa County, by and through its Department of Public Health (MCDPH), and St. Vincent de Paul for the provision of adult vaccinations. MCDPH will provide vaccine support, education on storage of the vaccine, and instruction on proper reporting. St. Vincent de Paul will administer the vaccine and complete the appropriate reporting. The agreement term will begin May 1, 2017

46. MOU WITH ST. VINCENT DE PAUL FOR ADULT IMMUNIZATION SERVICES Approve and authorize the Chairman to execute a MOU between Maricopa County, by and through its Department of Public Health (MCDPH), and St. Vincent de Paul for the provision of adult vaccinations. MCDPH will provide vaccine support, education on storage of the vaccine, and instruction on proper reporting. St. Vincent de Paul will administer the vaccine and complete the appropriate reporting. The agreement term will begin May 1, 2017 and expire April 30, 2020, unless terminated earlier as provided for in the agreement. There is no cost to the County or presenting clientele. The Immunization facility is located at St. Vincent de Paul, 420 W. Watkins Road, Phoenix, AZ 85003. (C-86-17-062-3-00)
