45. FACILITIES USE REVOCABLE LICENSE AGREEMENT WITH ST. MARY’S FOOD BANK ALLIANCE Approve and authorize the Chairman to execute Facilities Use Revocable License Agreement (FURL) P-50302, with St. Mary’s Food Bank Alliance, an Arizona non-profit corporation (SMFBA) for use of space at thirteen (13) Maricopa County Department of Public Health Women, Infant and Children (MCDPH-WIC) clinics. Authorize the Department Director, or his designee, as provided in Article 4.3 of the agreement, authority to change the time, date or location of the use so long as there is no cost to the County and the location is one of the facilities approved in the agreement. The agreement term will begin June 5, 2017 and expire July 31, 2017, unless terminated earlier as provided for in the agreement. Either Party may terminate the agreement with 30 days’ notice to the other. The FURL will allow St. Mary's’ Food Bank Alliance to provide a Summer Meal Service Program to youth under age 18 at the MCDPH-WIC facilities. The thirteen WIC facilities are located at: 1.
