47. AMENDMENT TO AGREEMENT WITH WESLEY COMMUNITY AND HEALTH CENTER FOR WIC SERVICES Approve and authorize the Chairman to execute amendment 1 to license agreement P-50301 with Wesley Community Center, Inc., an Arizona non-profit corporation doing business as Wesley Community Health Center (WCHC), for the purpose of providing WIC and other support services to families (Clientele) on WCHC premises located at rooms 2 and 3 at 1625 N. 39th Avenue, Phoenix, AZ 85009 (“New Premises”). The amendment will extend the term approximately two years from May 15, 2020 and expire on May 31, 2022. Effective March 1, 2020, the Old Premises, where services were provided, was moved from two rooms at 1625 N. 39th Avenue, Phoenix, AZ 85009 to one larger, unnumbered room, 416 square feet, at the same location. Consideration for the larger space during the extension will be $550.00 per month plus rental tax (24 months). There is no charge to presenting County clientele. The County will also remit to WCHC $550.00 for each month of March, April and May, 2020, plus rental tax. Though this is a 2 year term, rent is to be remitted for the 27 month period of ‘New Premise’ occupancy at $550 a month plus miscellaneous and tax (at 10%). Total compensation to landlord is not to exceed $16,335 for the term of the agreement. There is no charge to presenting County clientele. County may terminate this Agreement pursuant to A.R.S. § 38-511 or due to non-appropriation of funds at the end of County or Federal fiscal year. (C-86-17-066-L-01)

66. AMENDMENT TO AGREEMENT WITH WESLEY COMMUNITY CENTER, INC. Approve and execute Amendment No 2 to Agreement with Wesley Community Center, Inc. an Arizona non-profit corporation, doing business as Wesley Community Health Center (WCHC) for the purpose of providing WIC and other support services to families (Clientele) on WCHC premises located at un-numbered room consisting of approximately 416 square feet at 1625 N. 39t h Avenue, Phoenix, AZ 85009. The amendment will extend the term three (3) years from June 1, 2022 and to May 31, 2025. The monthly rent during the extension is $550.00 per month plus rental tax. For any reason or no reason and without consequence or liab ility, either party may terminate this Agreement upon sixty (60) days’ prior written notice. County may terminate this Agreement pursuant to A.R.S. § 38-511 or due to non-appropriation of funds at the end of County fiscal year. This action takes place in Supervisorial District No. 4 (C-86-17-066-L-02)

67. AMENDMENT FOR EARLY TERMINATION OF AGREEMENT WITH WESLEY COMMUNITY CENTER, INC. Approve and execute (P50301) amendment No. 3 to the agreement between Wesley Community Center, Inc. dba, Wesley Community Health Center and Maricopa County (County) dated May 10, 2017, as amended (Agreement) for early termination of Agreement. The parties agree to terminate the Agreement on February 29, 2024. County shall vacate the premises located at 1625 N. 39th Avenue, Phoenix, AZ on or before the termination date. Supervisorial District No. 5 (C-86-17-066-L-03)
