CONTRACT WITH UNIVERSITY PHYSICIANS, INC D/B/A UNIVERSITY OF COLORADO SCHOOL OF MEDICINE FOR JUSTICE-INVOLVED TOBACCO CESSATION PROJECT Approve a Request for Proposal (RFP) Contract (PH RFP 171182) between University Physicians d/b/a University of Colorado School of Medicine (CU Medicine), Justice-Involved Tobacco Cessation Project and Maricopa County by and through its Department of Public Health (MCDPH), for the purchase of tobacco cessation services for the justice-involved population in Maricopa County. The Contract is for one (1) year term beginning June 1, 2017, ending May 31, 2018. The RFP Contract amount is not-to-exceed $183,392. The County may, at its option and with the agreement of the Contractor, renew the term of this Contract for additional terms up to a maximum of five (5) years, (or at the County’s sole discretion, extend the contract on a month-to-month basis for a maximum of six (6) months after expiration). CU Medicine was awarded a contract under PH RFP 171182 on April 3, 2017, pursuant to MC1-330, Request for Proposal (RFP). (C-86-17-069-3-00)
